Completing your forms in advance helps us prepare for your visit and gives you more face time with Dr. Smith and the team — not paperwork.

As a fee-for-service practice, we place your health and the quality of your care above the restrictions of dental insurance contracts. While our office is not in-network with insurance providers, this does not necessarily mean you are without coverage. In many cases, out-of-network benefits are comparable to in-network benefits, allowing you to continue receiving the care you trust.
